Follow the instructions to request a payment from your annuity policy's guaranteed minimum withdrawal benefit (GMWB) income rider.

  1. Complete form

  2. Submit form
    Submit your documents according to the instructions on the form.
  3. Allow 5-10 days to process

    Please note: The processing time includes time to mail payment information to the policy owner.
  4. Disburse
    Payment will be mailed to the owner's address on record unless approved as a direct deposit payment.
